• In years past we were able to hold registration at Dr. Moser's office, and offer physicals on the spot.  Unfortunately, we will not be able to provide that this year.  Physicals will still be required, however, you will have until practices begin in August to hand in the form.  You may also mail them to the post office box if you wish.  Additionally, please provide a copy of your child's birth certificate.

Within 10 - 14 days the result will be sent to you via email.

Download form CY113 and print. This application must be mailed into the state with a copy of the Criminal background result and a $10.00 money order only. Results will be mailed to the applicant within 14 days.
A copy of the child abuse and background check should be given to the Twin Valley Football president before or during the first day of practice.  New Flag and new Might Mighty Mites coaches and assistant coaches may provide the criminal background checks to the president before the week two of the season.
